What is the complication of laser cosmetic operation?

How does the IPL system remove hair?

Hair removal with intense pulsed light (IPL)

Intense pulsed light (IPL) uses light to eliminate hair more effectively and for a long period of time.
How it worksThis technique is similar to laser epilation, the difference being that it doesn’t use laser rays but intense light given out by a flash lamp. By channelling this intense light, melanin, the pigment that colours hair, experiences a large increase in temperature. It transmits this heat to the hair follicle, liquefying it. It can be compared to laser epilation because it works the same way, destroying melanin, the pigment of the hair. It's particularly effective on dark hairs that are rich in melanin.
What it involvesBefore you think about undergoing IPL treatment, consult your doctor. It should be carried out by a trained practiitioner who is used to carrying out the procedure. Your skin should not be tanned when you have it done and should be shaved around 3 days beforehand.The area to be treated will be covered with an anaesthetising cream, and you should wear protective glasses, like the practitioner, during the whole epilation session.You will need to have several sessions several weeks apart to obtain the required result on the treated area.
Advantages- It’s an effective method, like laser epilation. After treatment, obstinate hairs become finer, rarer, and take longer to grow back. It’s a long-lasting treatment.- It’s practically painless: you’ll feel a little prickling or light burning sensation at most.

Rosacea responsive to intense pulsed light therapy

MedWire News: Intense pulsed light (IPL) therapy provides significant improvements in erythema and telangiectasia in patients with rosacea, say researchers in the UK, with results sustained for at least 6 months.
William Clayton (Royal Free Hospital, London) and colleagues conducted a phase III trial to evaluate the efficacy of IPL therapy in the treatment of 34 patients (25 female; mean age 47 years) with stage I erythematotelangiectatic rosacea.
Four treatments were administered on the face at 3-week intervals, using IPL 515-1200 nm, with a 560 nm cutoff filter. The fluence range was 24-32 J/cm2.
Significant reductions from baseline were achieved after the four treatments. The mean reductions in erythema values were 39% on the cheeks and 22% on the chin. Photographic assessment confirmed this, with a 46% improvement in erythema and a 55% improvement in telangiectasia.
The severity of rosacea was reduced from 7.3 points on a 10-poinnt visual analog scale at baseline to 3.8 points at the end of treatment.
The overall improvement in rosacea was assessed by both patients and physicians, with 73% of patients and 83% of physicians noticing a more than 50% improvement.
Writing in the British Journal of Dermatology, the researchers note that all of the results were sustained 6 months after the last treatment.
Adverse events were uncommon and self-limiting, with occasional bruising lasting for less than 1 week or edema lasting for 2-5 days. One patient discontinued treatment due to edema and crusting on the cheeks, but this resolved after 10 days.
The researchers conclude that IPL "significantly reduces erythema and telangiectasia with minimal unwanted effects, and results in high patient satisfaction."

Light Emitting Diode (LED)

LEDs are very small light bulbs which, in contrast to ordinary incandescent bulbs, don’t get especially hot and don’t burn out. These special lights are illuminated only by the movement of electrons in a semiconductor material, and they are very long-lasting. They were originally developed for experimental purposes -- namely to research the effect of light sources in photosynthesis for plant growth.
How does this apply to aesthetic goals? LEDs have been found to trigger natural chemical processes inside the cells, boosting the body’s own production of collagen, which slows in our late teens and, by age 25, nearly slows to a halt. This fact makes LEDs particularly useful for skin rejuvenation, because the treatments can wind back our collagen clocks, so to speak, producing collagen more like we did in our teens and early twenties (at least for a short period of time immediately following each treatment). Tattoo removal information

A tattoo used to be a permanent and irreversible adornment to one's skin. However, in recent years dermasurgeons have developed safe and effective techniques to successfully remove unwanted tattoos. (Read "Do's and don'ts for tattoos and piercings")


Patients request removal of a tattoo for a variety of reasons — social, cultural or physical. Some patients develop an allergic reaction to a tattoo several years after the initial application. Because each tattoo is unique, removal techniques must be tailored to suit each individual case. For instance, professionally applied tattoos tend to penetrate the deeper layers of the skin at uniform levels. This uniformity allows dermasurgeons to use techniques that remove broader areas of inked skin at the same depth.


Homemade tattoos are often applied with an uneven hand and their removal may be more difficult. Deeper blue and black ink colors are particularly difficult to remove. Professional tattoos made with some of the newer inks and pastel colors may also be difficult to remove entirely.


REMOVING TATTOOS

Tattoos can be removed by a dermasurgeon on an outpatient basis with local anesthesia. The most common techniques used are:



Laser surgery

The surgeon removes the tattoo by selectively treating the pigment colors with a high-intensity laser beam. Lasers have become the standard treatment because they offer a "bloodless," low risk, highly effective approach with minimal side effects. The type of laser used generally depends upon the pigment colors. In many cases, multiple treatments may be required.


Dermabrasion

The surgeon "sands" the skin, removing the surface and middle layers of the tattoo. The combination of surgical and dressing techniques helps to raise and absorb the tattoo inks. View the dermabrasion fact sheet for more information.


Surgical excision

The surgeon removes the tattoo with a scalpel and closes the wound with stitches. This technique proves highly effective in removing some tattoos and allows the surgeon to excise inked areas with great control.



ARE THERE SIDE EFFECTS OR COMPLICATIONS?

Side effects are generally minor, but may include skin discoloration at the treatment site, infection of the tattoo site, lack of complete pigment removal, or some scarring. A raised or thickened scar may appear three to six months after the tattoo is removed.

Intense pulsed light

Intense Pulsed Light (IPL), is most commonly employed by medical practitioners and aestheticians working under the guidance of a physician, is a method of hair removal from the body involving the use of a specially constructed xenon flash lamp and focusing optics.
The Intense Pulsed Light hair removal process has become very popular due to its cheaper and faster procedure compared to laser hair removal, which is often more costly and time consuming. The comparison of effectiveness between Intense Pulsed Light and Laser Epilation is debated by scientists, equipment manufacturers and practitioners, but results are generally accepted to be roughly equivalent. IPL is generally advertised as "permanent hair reduction" as opposed to "permanent hair removal" (these phrases are misnomers when applied to laser or IPL hair removal). It is also used for the treatment of the skin in a process known as photorejuvenation.
Hair removal mechanics: The focused, broad-spectrum light is applied to the surface of the skin by way of either a hand-held wand, or by an articulated arm. The intense light travels through the tissue of the skin until it strikes the hair shafts or the bulb (root) of the hair. The bulb is usually where the highest concentration of melanin is located, as opposed to the rest of the hair shaft. The light is converted to heat energy. The bulb and most of the hair shaft are instantly vaporized. The intense heat radiated by the hair also destroys the hair-producing papilla or the entire hair follicle. It is also claimed that direct light-heat conversion occurs directly in the darker colored capilaries that bring nourishing blood to the follicle.
IPL technology is also employed in the treatment of medical disorders of the skin including sun damage induced dyspigmentation and vascular changes; poikiloderma of Civatte; acne Rosacea; broken capillaries/telangiectases; vascular and pigmented birth marks. In terms of rosacea, IPL offers one of the few effective treatments to relieve the facial redness associated with the disorder. Such treatment is best administered by a specialist dermatologist. This new technology incorporates 'dual mode filtering' and other important advances which result in safer and more effective treatment than the older systems; which were initially designed for simple processes such as hair removal.
The pulses of light produced by IPL equipment are very short in duration, so discomfort and damage to non-target tissues is minor. Most people who undergo IPL epilation only experience slight irritation similar to that of a minor sunburn though under certain circumstances, blisters may occur. Men undergoing the procedure experience greater discomfort as male body hair is generally thicker than women's. Depending on the instrument settings and intensity, treating sensitive areas such as the chest and face can result in pain comparable to waxing the same areas; however this pain dissipates immediately. The light that emanates from the IPL wand is filtered to remove any ultraviolet components, eliminating the possibility of UV skin damage.

Necessary care after IPL / laser hair removal treatment

IPL / Laser hair removal, although it is a simple process, does require proper care of the skin in the area that was treated. Generally there is little recovery time needed regarding IPL / Laser hair removal procedures. People can usually return to normal activities immediately after treatment. However, it is important to take the time to follow the recommended guidelines for aftercare in order to protect against any chance of a complication and to reduce any discomfort that may be experienced.

Immediately after the IPL / Laser hair removal treatments, there should be redness and bumps at the treatment area, which may last up to 2 hours or longer. It is normal for the treated area to feel like a sunburn for a few hours. Some physicians recommend aloe vera gel or some other after-sunburn treatment such as Desitin. Darker pigmented people may have more discomfort than lighter skin people and may require the aloe vera gel or an antibiotic ointment longer.
Makeup may be used after the IPL / Laser hair removal treatment, unless there is epidermal blistering. It is recommended to use new makeup to reduce the possibility of infection. Just make sure that you have moisturizer on under your makeup. In fact, moisturizer frequently and freely on the treated area. Any moisturizer without alphahydroxy acids will work.
Avoid sun exposure to reduce the chance of dark or light spots for 1 month. Use sunscreen SPF 30 or higher at all times throughout the treatment.
Avoid picking or scratching the treated skin. DO NOT USE any other hair removal methods or products on the treated area during the course of your IPL / Laser hair removal treatments, as it will prevent you from achieving your best results.
You may shower after the IPL / Laser hair removal treatments, and use soap, deodorant, etc. The treated area may be washed gently with a mild soap. Skin should be patted dry and NOT rubbed. Underarm areas, that have been treated, should be wiped with alcohol for 24 hours. You may apply deodorant after 24 hours. Wait two (2) days before shaving treated areas.
Anywhere from 5-30 days after the treatment, shedding of the hair may occur and this may appear as new hair growth. This is not new hair growth, but dead hair pushing its way out of the follicle. You can help the hair exfoliate by washing or wiping with a washcloth.
Hair re-growth occurs at different rates on different areas of the body. New hair growth will not occur for at least three weeks after treatment.
Call your physician’s office with any questions or concerns you may have after the treatment.

Please note: Stubble, representing dead hair being shed from the hair follicle, will appear 5-30 days after the treatment date. This is normal and will fall out quickly.

What pre-treatment is required before receiving IPL?

There's really no pre-treatment that goes into IPL, or Intense Pulsed Light. However, you need to be careful about going into the sun prior to the IPL treatments because the sun can obviously either lighten or darken your skin. And the IPL machine can either lighten or darken your skin. So you really need to be careful about being tanned before having an IPL treatment. You should check with your MediSpa and check with your doctor in terms of how long they'd like you out of the sun prior to the IPL treatment.

Can I receive IPL treatments if I am on medication?

There are a few medications where I and other doctors should have concern. Certainly if you're taking a skin bleaching or changing agent for your skin, you need to be careful before you have IPL or any treatments. The other area is whether or not you're taking Accutane, that's the drug perscribed for persistent acne. It's important that you stop taking Accutane before having IPL or any other laser treatments.

What is IPL acne treatment?

One of the more exciting treatment options with the IPL or Intense Pulsed Light machine these days is its treatment within the acne regimen. Now for many years the only way acne was treated was with topical agents or with Accutane. More recently, and with the development of the Intense Pulsed Light machine, acne is now treated by IPL treatments. How does it work? Specifically the IPL targets the sebaceous gland where the oil is secreted and really just zaps that area. By getting rid of the sebaceous glands and the oil, obviously the acne is diminished. Now, it's not going to permanently get rid of all of the acne glands. It probably lasts about three or four months, but for patients that either have adult acne or don't want to take Accutane it's a nice treatment that we can offer now.

How many procedures are available with IPL?

What's made Intense Pulse Light or the IPL machine so effective is the fact that it's one machine with a variety of different hand pieces which allow it a variety of treatment options. Depending on the machine, it can do hair removal, it can do photo-facials, or skin rejuvenation, it can do spider veins, it can also take care of acne. Again, some of that depends on the actual machine and the hand pieces. But that's why people like IPL because it has many different options.

Who is authorized to administer IPL?

An IPL machine is not a laser. In many states and in many spas it is important, and it's regulated, that the doctor perform the laser treatments. Again, you're going to need to check your state. With an IPL machine, because it's not a laser, the regulations are really a gray area. So again, state to state it depends who can use an IPL machine. It can be either your doctor or the aesthetician or somebody in the MediSpa who uses the IPL machine. In any regards, each IPL machine should have its own registration accreditation where whosever is using that IPL machine should know how to use it. And you should ask in the MediSpa to make sure that those accreditations to use an IPL machine, that certificate is available.

What are the risk factors for IPL?

What's good about IPL is it doesn't have the invasiveness of a typical laser, which means the concerns about bleeding and scarring are significantly less. On the other hand, for any patient that is undergoing this treatment, you need to make sure that you don't have the risk of either hyperpigmentation, which is darkening of the skin, or hypopigmentation, which is lightening of the skin. Specifically, African Americans or dark-skinned patients, you need to be very careful before you use an IPL machine. In fact, there are very few instances where you want to use an IPL machine. So the risks are almost entirely pigment changes. That's why it's important, if you're going to a medi-spa, that the doctor examines you before anybody does any treatment.

What is the difference between intense pulsed light treatments and traditional laser therapy?

Lasers have been around for a longer time than the IPL machine, and the difference is that the laser usually targets one small area of light to target a specific part of the body, whether it's hair or veins or skin. It can be very invasive, and doctors do need to use the lasers. The IPL machine, because it uses a broader spectrum of light, is not as invasive, and will cover a broader area of treatment options. Whereas you may need one laser for hair, one laser for skin, and one for veins, an IPL with different hand sets can treat all of those different areas. The other issue is that because the IPL is not a true laser, the regulations of who can and cannot use the IPL machine are a lot different, and state-to-state dependent.

What is "Intense Pulsed Light" (IPL)?

IPL, or Intense Pulsed Light is similar to a laser, but it has some noticeable differences. First of all, a laser usually specifics one type of area whether it be skin, or blood, or water. The Intense Pulsed Light, or IPL targets many different parts of the body, and can be used effectively in different cases such as hair removal, photo facials, or even taking care of small spider veins. The IPL machine is used very commonly in a Medi-Spa, and there are a variety of reasons why. One, it's safe, effective, and has many uses, and two, depending on your state, sometimes a doctor is not required to actually use the IPL machine. That's different to a laser. In many states if you're going to have laser machines, or any laser treatments, they need to be done by the physician. In many states, with the IPL machine, because it's not considered an actual laser, the treatments can be performed by either an aesthetician or someone else in the spa.

Laser Hair Removal or IPL - What is the Difference and Which is Best For You

Laser and IPL have been used to remove unwanted hair for many years with a high degree of success, however, to be actuate we must call it "Permanent Hair Reduction." According to FDA rules manufacturers may not claim that laser (or IPL) hair removal is permanent unless the FDA determines that there is sufficient data to demonstrate such results. Several manufacturers have received FDA permission to claim, "permanent reduction," but NOT "permanent removal."
What this means in practice is that although most hair will be removed over a course of treatment, some hair will remain and some may re-grow, usually much lighter and finer, therefore, occasional top-up treatments may be needed.
Laser and IPL use energy in the form of light to destroy the hair follicle. The energy directed from the handpiece is absorbed by the melanin in the hair follicle, momentarily heating it to around 70 degrees centigrade killing the follicle and thus preventing further hair growth. However to complicate matters most people also have melanin in their skin that also absorbs light energy creating heat in the skin. If this heat is excessive it will cause blisters or burns. For this reason the ideal candidates for these treatments are those who are dark haired and fair skinned like the typical Irish complexion, whereas suntanned Danes with very fair hair are poor candidates.
We always insist that clients stay out of the sun for a few weeks prior to treatment so that the level of melanin in the skin is as low as possible and therefore wintertime in the UK is the ideal time for treatments.
With care most people can be treated but some may require many more treatments and any white, grey and even some red hairs will respond poorly (or not at all) to light based hair removal treatments. These will require many more treatments at a much lower power level and the operator will have to be extremely careful to ensure that the machine is correctly set to avoid skin damage.
Thankfully, most modern laser and IPL devices make this process much easier but it is still necessary to have a highly skilled operator and, here in the UK, all establishments must be registered with the Health Care Commission before they perform any laser or IPL treatments.
Hair grows in cycles and many factors influence it's growth including age, ethnicity, weight, hormones, diet, medication and metabolism along with your hair's location, thickness and resilience. Only hair that is in it's growing cycle will be destroyed at each treatment session. The number of hairs growing at any one time and the length of time between treatments varies, but generally we allow 4 to 8 weeks.
There is sometimes mild pain associated with laser treatments. This discomfort ranges in intensity and has been described from as mild as the snap of a rubber band, to the sensation of a hot pinprick. When treating sensitive areas, (upper lip and bikini areas) some practitioners often elect to use a topical anaesthetic cream, which helps to reduce any discomfort.
There are two big differences between Laser and Intense Pulsed Light systems. One is in the area that can be treated in one go and the treatment speed. Generally IPL treatment heads are up to 8 times larger than the small spot produced by lasers and therefore treatments are much quicker.
The other difference is the wavelength of light produced. Lasers produce light with one specific wavelength whilst IPL uses an entire waveband. Different wavelengths penetrate the skin to different depths so using IPL is like using a group of lasers in a single treatment.
In our clinic we use an Ellipse Flex that with I²PL technology employing dual-mode filtering which removes water absorbing wavelengths from the output spectrum of the system. With the older intense pulsed light technology, water absorbing wavelengths are absorbed in the skin creating a non-specific thermal injury, which can lead to an increased risk of side-effects particularly burns.
To remove the infrared light, the Ellipse I²PL™ system has a unique extra filter consisting of circulation water. This means that the light components that normally would “heat” the water in the skin now “heat” the water in the system instead. By eliminating these wavelengths, the Ellipse I²PL is able to more specifically generate heat in the hair follicles rather than the skin. The unique Ellipse I²PL™ Technology eliminates the unwanted wavelengths, and standardizes the light use, meaning less energy is required to achieve a clinical effect. As a result, pain is reduced, while minimizing side-effects. With this new technology there is no need for thick, messy gels and contact cooling which makes the patient's experience more pleasant.
Two classes of IPL systems are available; those that HAVE been tested for safety and performance by accredited, independent testing facilities, such as the European Union Medical Devices Directive (EU), as well as the FDA (Food and Drug Administration — US Government), and those that HAVE NOT been subject to such third-party testing.
Such untested systems are labelled with the standard CE mark but without an EU number. A system with a CE or FDA accreditation provides certainty that the system has undergone and passed stringent safety tests, that the manufacturer has documented that the system can perform the stated clinical treatments efficiently, and that possible risk and side effects are of an acceptable level for the results obtained.
The ELLIPSE IPL systems, have obtained accreditation and approval from both the EU as well as the FDA.
For your own safety when deciding on your treatment ensure that you choose a clinic that is registered with the Healthcare Commission, if not they will probably be un-insured, that uses a well known, tested and proven device. You must have a comprehensive consultation and patch test several days before you undergo any treatment and make sure you are not seduced by outlandish claims or discounts.
